Thank you for participating in the upcoming exhibit and online blog show.

Please read the following information, sign, date this form and include it with your quilt when you drop it off or mail it to the church.  All quilts, releases and information forms must be received no later than 5:00 PM on Friday, April 10, 2015

I hereby grant permission for my Glorify Jesus Christ quilt to be displayed at Fairfax United Methodist Church from Friday, May 8 through Monday, 11, 2015, and to be photographed with said photo and documentation including my name and personal statement in part or in entirety to be displayed at the show, as well as on a virtual Internet quilt show in perpetuity.  I understand that the actual work of art is my sole property and I am solely responsible for insuring my quilt.  Debra Godwin, Event Coordinator nor Fairfax United Methodist Church of Fairfax, Virginia, will be held liable for any damage, loss or theft of my quilt during any part of this event to include transportation, exhibition and storage.

Quilts will be available to be picked up at the church on Monday, May 11, 2015 from 9:00 am to 9:00 PM unless special arrangements have been made in advance.
